Student Affairs "ER" Form (Emergency Request Form)
The Office of Student Affairs at EOU acknowledges that emergency circumstances sometimes arise, often beyond your control, which may prevent you from being able to complete your coursework. If you are in need of additional time because you have recently experienced a major medical crisis, death in your immediate family, or an emergency/crisis situation, we want to help! *** Procrastination is not considered an emergency circumstance! *** Because of our commitment to helping our students in the best way possible, especially during times of crisis, please be aware that you will not be able to register and/or take additional distance education classes at EOU until all of your coursework is completed and final grades for each class have been posted. In order to process your Emergency Request, please do the following: Step 1 - Complete and submit the online "Emergency Request" form below. Note: This request is only valid for the current term, not for courses taken in any prior term. You must submit this form before the end of the eighth week of the term. Step 2 - Send some form of documentation from an appropriate professional source (e.g. Health Professional) which specifically addresses & verifies your circumstances. Please send the documentation via postal mail, email or fax to: Student Affairs "Emergency Request" Coordinator Office of Student Affairs Eastern Oregon University La Grande, OR 97850 Phone: 541-962-3476 Fax: 541-962-3924 Email: saffairs@eou.edu Upon receipt of your online "ER" form AND accompanying document(s), we will then forward your emergency request information to your instructor(s). In conjunction with them, we will make a decision about whether the request is approved and the period of time specified for the completion of your coursework. Once a decision has been made, you will be contacted via e-mail at the address you list on the form below. Further, if you are on financial aid and your request for additional time is approved, we will send a copy of the approval notice to the EOU Financial Aid Office to be placed in your file. You need to be aware that you must still meet the satisfactory academic progress standards. The approval of this extension may mean that you will not meet those standards. #1. The information above is correct and accurate.
#2. Documentation from an authorized source will be sent to the DDE "ER" Coordinator to verify your circumstances.
#3. An administrative hold will be placed on your EOU Webster account which will prevent you from taking additional EOU distance education courses. Once all of the classes listed above have been completed and a final grade for each has been posted, the hold will be lifted and you can then register for additional EOU distance education courses.
Remember: DDE must receive your documentation from an appropriate professional source (e.g. Health Professional) to verify your circumstances prior to processing your request.
